As described in the Web and Mobile Terms and Conditions, we may obtain information related to your prescription benefit plan and pharmaceutical services.

Registration/Account Signup Information. We may collect certain personal information including your first and last name, postal address, phone number, email, gender, date of birth, login credentials, information related to your employer, the payment of prescriptions, and prescription benefit plan and certain medical insurance plan information.

We collect information about you (and your dependent, if applicable) when you provide contact and identity confirmation information to us via email, mail, or through the Site, including as a result of creating an account, signing up for communications, response to surveys, requesting support or any other communications. Information We Get From Others. We may obtain additional information about you from third party sources to provide you with more relevant information about our services.

The Site is not intended for use by anyone under the age of 18. We do not knowingly collect or solicit personal information from anyone under the age of 18 unless such personal information is provided by the parent or legal guardian of a dependent under the age of 18. If you interact with any third party, including through any application or content (including advertising) that may link to or be accessible from or through the Site, we are not responsible for any such third-party websites, content or services. Do Not Track Disclosure
Some third-party sites do keep track of your browsing activities when they serve you content, which enables them to tailor what they present to you. We do not track Site users over time and across third party websites to provide targeted advertising and therefore our Site does not respond to Do Not Track (DNT) signals.
